How to Obtain Vital Records
To request a vital document, simply bring your driver’s license or state-issued ID card to one of our currency exchange locations. If you’re under 18, you will need a parent or guardian with a valid form of identification to accompany you.
To request a birth certificate for yourself or your child, you’ll need to provide the city of birth as stated on the certificate, the complete name, the precise date of birth, and the first and last names of the parents.
After we receive your request for records, we will contact the Cook County Department of Vital Records. The documents will then be shipped to the currency exchange location where you made the request. It typically takes 7-10 business days to arrive. We are unable to ship documents directly to your home address, but with our convenient hours, you can easily pick up your documents on your schedule.
